Students must have a complete registration online via the NEW FinalForms program, have a current physical (within 2 years) on file at the high school and a current Health Care Plan (if necessary) in order to participate in a sport.
The Mead School District requires all incoming 9th graders to obtain a current physical due to the intensity of high school athletics (after June 1 and prior to their first practice. Sports Physicals are then good for two years, after their freshman year, from date of exam.
Regarding new physicals: Submit the form, dated & signed by the physician, either to the school nurse or to the Athletic Department. Physical Evaluation Forms are available to print from FinalForms and the school’s webpage.
